Hi Team, I am looking to create a custom action which would convert RISK into an Issue, the workflow is not working as expected, can anyone help with that?

You don't really convert it. The CA should create a new issue, link it to the project, update the state of the RISK to a closed or completed state. I also add discussion posts to the closed risk and new issue so that you can cross reference or track the history of the case
